NFA – Change of Address With Pending Form 4

Q: I have a customer that we sent in the form 4 for several months ago and we got back a letter with changes that needed to be made, this was only the second one that we had sent in so there are a few things that we didn?t know we had to do. One issue was I had sent in a photo copy as the duplicate form, AFT ask for that both copies be in original ink, with original photo. So I called the customer to come sign a new copy and for him to go back to the sheriff and have the sheriff sign the form as well since the duplicate was a copy not original. At this point our customer informs me that he has moved and lives in a different county. He now lives in Montgomery County instead of Clark County. The question is do we leave the old address on the form 4 or change to the new one if we change it to the new one the sheriff in Montgomery County will not sign off on form 4?s and our customer will have to move everything to a trust. If he does that will it start the process over or will the case number that has been assigned to the first form 4 carry over to the to the trust.
