Form 4473 – Missing Info for Trace Request

Q: We received a trace for a firearm sold in 2011. We found the form, but information for questions 3-7 is missing. Would be better to get the information from purchaser and attach letter to ATF telling them he got the information from the buyer or just go ahead and fax the form as is?
